I had this issue as well and overcame it.

I bought two of the longer nuts like these compare.ebay.com/like/170840202777?var=lv<yp=AllFixedPriceItemTypes&var=sbar

I then used a dremel to cut a fine groove in the ends of both nuts and using piano wire, I tied the two bolts to each other so they couldnt vibrate loose; never came off again.
