For the most part, it's for real.
The 'Plus' in MagicjackPlus is the new ability to plug the magicjack directly into your router connection, whereas before you could only plug it into a computer's usb port.
It's relatively cheap, but not free. You do pay for the box itself and then annually for the service. I think the box is around 70 bucks and the service runs in the neighborhood of 15 bucks a year.
While international calling is free from other countries to the states, outbound international is not. I've seen folks setup a magicjack here, then take it overseas to call home while they're away. Again the only stipulation is you must have an internet connection.
Some have had bad things to say about it, mostly setup issues. My cousin has had magicjack for years, and swears by it.

And if I get one and get rid of the local phone co,. then I don't have internet! oooops So true John you read my mind........but if you had lots of buddies that required long distance charges at only $15 a year it would even be cheaper than a cell phone.

Yeah, in the phone world we call it 'naked' dsl. It's dsl only, with no working dial tone. I've had it for years. I used Vonage for awhile, then even gave it up. i just haven't needed a home phone. All my calling is cellular.
Google Voice is another alternative to Magicjack.

John I pay with tax $62 for hughesnet satellite internet only. They just put up the new Gen4 satellite and you can pick one of 3 options on the speed. The lower one which is what I got is 2X faster than any land line dsl.
Their top tier is the at the moment fastest thing on the planet.

Magic Jack--I have one now for about 4 years and we like it--at first it cost us 39.95 for the setup and 19.95 each year thereafter ---until this year ---went up 10 dollars but still 29.95 a year is pretty cheap--btw--if you got the regular Magic Jack , you can take your phone and laptop to a WIFI hotspot ,like at Mc Donalds and use it there --crazy--btw--if you got the new Uverse Internet (fiber optics) you don't need a phone line anymore--and what Ma Bell don't tell you is if you got DSL you can ditch your land line ,as well...and just use Magic Jack--as for the magic Jack PLUS,it does not need a usb connector on a puter ,it just hooks into your router, just like any puter ,by ethernet--it costs about 100 bucks for the initial setup ,so I don't see any real need for it ,since you got Internet ,you already got a puter with a usb connector...I couldn't see any reason to buy the new Plus system for the higher price, since I am already paying for Internet and got a computer...so,here is my story ,I got the new Uverse Internet service (fiber optics) (12 MPS)in my neighborhood, and ,ditched myold expensive land line and use only Magic Jack and a couple cell phones, which I was already using anyhow--I know this is a long winded post but I thought I should clarify the situation...or muddle it up for some folks-lol--any questions ,I be glad to answer --fflintstone

Others have covered most of it very well, the only thing I would add is that I know a few folks on here use satellite based internet. MagicJack will work on this but there will be a significant lag on calls as your call will be routed via space and back It should still work but less wonderfully. Personally I've used a few similar things over the years and after a bit of bashing to get them working they worked well. Skype works amazingly well for me as I can pickup calls and video call on my cell phone as well as in the house on computers etc.
